Objective: The purpose of this study was to determine postoperative pain after root canal therapy performed with hand files versus R-Endo rotary instruments. Methods: Eighty single-rooted teeth were included in this study. Patients without preoperative pain were randomized by minimization program into two groups. The standardized protocol for all teeth involved local anesthesia, isolation, access and irrigation with 2.5% NaOCl. Root canal fillings of the teeth in group 1 were removed by Headstroem files; and, in group 2 removal was performed via R-Endo rotary instruments. Both groups were medicated with calcium hydroxide and then closed with a temporary filling material. At 4, 8, 12, 24 hours and 2, 3, 4, 5, 6, 7 days, patients recorded postreatment pain levels. Data were statistically analyzed using the chi-squared and Fischer exact tests. Results: There was a statiscal difference in postoperative pain incidance between the groups. Postoperative pain levels were significantly higher at 4th hour and 1st, 5th, 6th and 7th days in hand file group. There was no significant difference in the occurrence of flare-ups between the groups. Conclusion: R-Endo rotary system was found to be effective in reducing postoperative pain in retreatment cases. Amaç: Bu çal??man?n amac?, retreatment için geli?tirilen ilk Nikel-Titanyum döner alet sistemlerinden biri olan R-Endo ve geleneksel el aletleri ile gerçekle?tirilen retreatment i?lemini, postoperatif a?r? yönünden kar??la?t?rmakt?r. Gereç ve Yöntemler: Bu çal??maya 80 adet tek köklü di? dahil edildi. Preoperatif a?r?s? a?r?s? bulunmayan olgular minimizasyon yöntemi kullanarak randomize edildi. Lokal anestezi, izolasyon, giri? kavitesi aç?lmas? ve %2.5 NaOCl ile irrigasyon ile tüm di?ler için protokol standardize edildi. 1. Gruptaki di?lerin kök kanal dolgular? hedström e?eler ile, 2. Gruptaki di?lerde ise kanal dolgusu R-Endo döner alet sistemiyle uzakla?t?r?ld?. Her iki grupta da kalsiyum hidroksit medikamasyonu uygulanmas?n?n ard?ndan di?ler geçici restorasyon materyali ile kapat?ld?. 4., 8., 12. Ve 24. Saatler sonras?nda ve 2., 3., 4., 5., 6. Ve 7. günlerde hastalar postoperatif a?r?lar?n? kaydettiler. Veriler istatistiksel olarak ki-kare ve Fischer gerçeklik testi kullan?larak analiz edildi. Bulgular: Postoperatif a?r? insidans? her iki grup aras?nda anlaml? olarak farkl? bulundu. El aletleri grubunda Postoperatif a?r? 4. saatte, 1, 5., 6., 7. günlerde anlaml? olarak yüksek bulundu. Flare-up aç?s?ndan guruplar aras?nda istatistiksel olarak anlaml? bir fark bulunmad?. Sonuç: R-Endo döner alet sistemi retreatment vakalar?nda postoperatif a?r?y? azaltmada etkili bulunmu?tur.
